• US citizenship or equivalent
• PharmD degree from an ACPE-accredited college of pharmacy
• Eligibility for Illinois licensure
• Letter of intent
• Curriculum vitae
• Three (3) letters of recommendation (two (2) from clinical preceptors preferred)
• College transcripts
• On-site or virtual interview
